The way she smiles and looks when he is in pain when transforming gives me the creeps, I think she is a bit of a sadist of some kind or gets off on people in pain
She’s also being encouraging to him. He is not in danger and while it hurts she mostly wants to motivate him to push through it and not seem troubled by the entire thing. Sara loves Alex and if she felt he was in real distress she would drop her façade pretty quickly. She can be a bit of a bully for sure though.

She has a lot more experience with it so I’m thinking it might be that she’s wondering if he’s putting on a show for her or if the ‘change’ affects him differently than it does her, and if it might get better or easier with time.
Fun comic with a fun approach to werewolves as an everyday thing, it reminds me a little of ‘Peter is the wolf’ while still being completely different.

I know there’s a bit of talk between them and 1 page where she rips open the car door
But it would be cool to see that scene portrait in a few pages call it a prequel?
